Introduction
Assumption Ledger is a function-hooks mod from ModSmith. It registers a register_assumption tool so Claude records assumptions, decisions and considered-but-not-done items, shown above the prompt by kind after the turn, with a Do it button for skipped items. It makes no model calls of its own; the ~200-token tool schema rides along each request. /assumptions write [path] appends to DECISIONS-log.md; nothing is written otherwise. /assumptions off stops recording and hides the tool.
